The ELHAP Transition Project is for disabled young people, who are moving on from children’s services. At the end of the Project, young people will be able to make informed decisions about their future including jobs, study, interests, hobbies and friendships. This project is most suitable for those who need support with their transition from childhood to adulthood. Currently, the focus is on young people who do not receive other funding and who would most benefit from exploring work and college opportunities.
